PWD ID

What are the requirements for a PWD ID? 

 

1. A valid, government-issued ID showing proof of residency in Cainta;

2. Medical Certificate indicating disability; 

3. Latest photo in 2x2 size, 2 pieces; and

4. Signed PWD Application Form (click to download form)

 

SENIOR CITIZENS ID 

What are the requirements for a Senior Citizen’s ID? 

 

1. A valid, government-issued ID showing the date of birth and Cainta address; OR

    Barangay Indigency Certificate or Birth Certificate 

2. For transfer of registration, Certificate of Cancellation from the previous LGU;

3. 1 x1 size picture

SOLO PARENTS ID

What are the requirements for a Solo Parents ID? 

 

1. Original Barangay Certification that the applicant is a resident of the said barangay for the last six (6) months; 

2. Photocopy of appropriate documentation/evidence that the applicant is a solo parent (e.g. death certificate of spouse, declaration of nullity of marriage, medical certificate of spouse if incapacitated, Solo Parent affidavit (click to download form); 

3. Photocopy of latest Income Tax Return (ITR) or any document that will establish the income level of the solo parent, including latest payslip or a Certificate of Employment stating the monthly income; 

4. Latest photo with white background, 1 x 1 size;

5. Photocopy of any ID indicating Cainta as residence; 

6. Birth certificate of children (those living with and dependent upon the solo parent for support who are unmarried, unemployed, and not more than 18 years of age, or even those 18 years old but are incapable of self-support because of mental and/or physical defect/disability) 

7. Marriage Certificate for those married, or Birth Certificate for those single and latest CENOMAR.
